NHMMI Saves Elderly By Implanting LOT CRT-D For The First Time In Chhattisgarh

NHMMI has saved an elderly by implanting a life-saving device LOT CRT-D with LBP – Lifesaving procedure for Heart Failure patient, thats too for the first time in Chhattisgarh.A group of expert consultants of NHMMI have done this feat.
Heart diseases are more common among the elderly, but the difficulty level increases in complex procedures performed on them. When the condition is heart failure and heart rhythm does not improve even after a bypass, and they require regular hospital admissions, the constant threat to their lives can disturb the entire family’s routine.
Heart failure is a condition where the heart is unable to pump blood sufficiently, meaning blood doesn’t circulate appropriately to supply the body with the oxygen and nutrients it needs to thrive. A weakened heart results in inadequate blood supply to the cells leading to fatigue, shortness of breath and other symptoms of heart failure.
Here is a case of a 65-year-old man who had undergone a bypass surgery long back, despite of which the pumping of heart was not proper. He was required to get frequent hospital admission due to heart failure. The risks included unimproved heart rate, unimproved heart pumping, and complete heart blockage leading to a possibility of sudden death.
Dr. Sunil Gouniyal, Sr. Consultant – interventional cardiology (NH MMI Narayana Superspeciality Hospital, Raipur), chose a device called LOT CRT-D (Left Bundle Branch-Optimized Cardiac Resynchronization Therapy Defibrillator) to treat the condition that combines left bundle pacing with CRT & defibrillator. LBP is a newer advancement in pacing that is more physiological (feasible and safe) and has more long-term benefits. The complex procedure was successfully performed by Dr. Sunil Gouniyal with support from Dr. Rakesh Chand (Sr. consultant cardiac anaesthesia and HOD Anaesthesia), Dr. Arun Andappan, Dr. Dharmesh Lad, Dr. Rupa and the entire Cath lab & ICU team.
A LOT CRT-D is a device for heart failure patients at high risk for sudden cardiac death. Functioning like a normal pacemaker to treat slow heart rhythms, a LOT CRT-D device also delivers tiny electrical impulses to the left and right ventricles to help them contract in sync. The action will help the heart pump more efficiently. A LOT CRT-D device can also treat dangerously fast heart rhythms (arrhythmias), which can lead to sudden cardiac arrest. If the device senses dangerously fast heartbeats, it gives an electric shock to the heart. This shock (defibrillation) stops the abnormal rhythm. Without this life-saving therapy, the dangerously rapid rhythm could lead to death in just minutes.
Dr. Sunil Gouniyal explains, “This patient came to the emergency unit with symptoms of heart failure and low heart rate due to complete heart block. When we realized that medicines were ineffective and he had already undergone a bypass surgery earlier, we decided to take the approach of LOT CRT-D. The patient is now doing fine and has an improved clinical condition.”
