Heart is a very complicated organ and furthermore complex it understanding the functioning and rhythm of heart beat. When heart rate doesn’t increase during high physical activity can be a sign of a dangerous disorder, it could even be a heart block. Here is a case of a child who suffered from Complete Heart Block (CHB).
Complete heart block, is a serious cardiac condition in which the heart rate of a person remains low and doses not increase to appropriate level when needed- like walking, running etc. Such condition happens because of fault in the conduction system (electric circuit) in our heart. Normally in children it happens due to congenital (by birth) or sometimes after cardiac surgery. about 1% of congenital cardiac surgeries may result in heart block. Untreated cases of complete heart block, especially when the condition is a result of cardiac surgery, have a very high risk and could even result in death (high mortality rate).
A 5yr old baby girl with a weight of 14 kg was suffering from Tetralogy of Fallot (Blue baby syndrome). Blue baby syndrome is a condition that cause cyanosis (Bluish or greyish colour of the skin, nails, lips or around the eyes) in babies because of low oxygen levels in the blood. The Cardiac sciences team at NH MMI Narayana Superspeciality Hospital, Raipur has been successful in restoring the health of the baby. However, post-surgery she developed complete heart block. Despite waiting for equate time (more than 2 weeks) it did not improve. Dr Sumanta Shekhar Padhi (Sr Interventional Cardiologist) proposed a complex yet effective solution – Left bundle pacing.
Pacemaker implantation is the usual treatment of choice in such cases. Left bundle pacing is a newer advancement in pacing that is more physiological (feasibility and safe) and has more long-term benefits. However currently the option is more feasible for adults due to varied technicalities involved, and no dedicated instruments are available for children. Hence such kind of procedures for children are done in very limited centers and needs very high levels of skill and expertise.
Left bundle pacing involves permanent pacemaker implantation. The Team of doctors led by Dr Sumanta Shekhar Padhi (Sr Interventional Cardiologist) with the help of Dr Kinjal Bakshi ( Sr Paediatric cardiologist) and Dr Dharmesh lad ( Sr Cardiac Anaesthesiologist) the procedure was done successfully on Dec 31st.
Dr Sumanta Shekhar Padhi (Sr Interventional Cardiologist) Said, “Post procedure the child had a normal recovery and was discharged in 5 days after the procedure. This was one of the rare case of such young kid to undergo this procedure in India and she was the youngest kid in central India to undergo this procedure.”
